Advance Directives

Ozarks Healthcare supports your right to make health care decisions; and in order to do so, you have the right to formulate a health care treatment directive, such as a living will or durable power of attorney for health care. These documents express your choices about your care or name someone to decide if you cannot speak for yourself.

Ozarks Healthcare provides forms called "Durable Power of Attorney for Health Care Choices and Health Care Choices Directive" when you are admitted to a hospital service or to persons interested in more information.

Ozarks Healthcare will honor your advance directive as it applies to your condition. You will be asked if you have a directive when you are admitted to a hospital service. If you have an advance directive, a copy will be placed in your medical record. The directive may be updated anytime you wish.
