Ozarks Healthcare Partners with Community Care Providers to Ensure Seamless Transition of Care
Ozarks Healthcare is excited to announce a collaborative effort with Assist America Home Services, Gentiva and all other organizations who provide community care across its seven-county service area in Missouri. This initiative is aimed at providing comprehensive support, including housekeeping, grocery shopping, and personal care services, for patients in Douglas, Howell, Oregon, Ozark, Shannon, Texas, and Wright Counties.

With a strong focus on patient wellbeing, Ozarks Healthcare At Home is dedicated to ensuring that patients continue to receive the care they need during this transition period. The process to secure a new in-home provider is simple, and patients are encouraged to take the following steps:
- Contact Missouri Division of Senior and Disability Services: Patients can promptly reach out to the Missouri Division of Senior and Disability Services at 1-417-895-6455 to request a new in-home provider.
- Seamless Transition: Within the next 30 days, a member of the Ozarks Healthcare Care Team will reach out to patients to ensure a seamless transition of care. The goal is to match patients with the right provider that suits their specific needs.
- Continued Ozarks Healthcare At Home Support: Ozarks Healthcare At Home remains dedicated to providing community care services and will be available through October 31, 2023. During this time, patients have the opportunity to arrange for another in-home provider, and the OZH Care Team is prepared to discuss cases with Division of Aging case workers to facilitate the transition of care.
